Demystifying Course Records |
Deciding which course record sheet (a.k.a. CRS) to use and remembering how to fill it out properly are challenges we understand! We have developed this guide as a refresher for all of you instructors. We hope it helps!

Directions for submitting course records:
- These forms can be submitted via email, fax or in person.
- When using email to send your course records, remember to submit all Authorized Provider (AP) Course Records to Judi Backof.
- For fax submissions to:
- Miami – 305-644-1038
- Broward – 954-797-1861
- Homestead – 305-248-5141
- Be sure to use the correct form – there are many to choose from – Water Safety Instructor's Activity Report for swim lessons and water safety levels 1-6, Lifeguarding classes, Course Record and Course Addendum for all other First Aid and CPR classes and HIV/AIDS Education Instructor Activity Report for HIV/AIDS classes.
- Know your class codes – this is very important for us to issue the correct certificates to your students. The current list of class codes can be found in the same location on www.redcross.org as the activity reports and course records.
- Don't forget to indicate how you would like to receive your certificates, if applicable. Do you want them mailed to you, mailed to the agency you taught for or would you like to pick them up? There is a place to indicate your preference on the forms.
- Remember that the agency or person listed as the Authorized Provider is the agency or person that will receive the invoice for any program support fees or materials rented/bought. Instructors are not always the contact for the AP agreement and billing, so please be sure to make that distinction.
- On the Course Addendum:
The only appropriate markings under "Enrolled" are E for enrolled, N for not enrolled (in a specific course component) or X for no-shows.
- The only appropriate markings under "Grade" are P for passed, F for failed, I for incomplete and A for audit.
